New Patients:

Bring a list of all current (over-the-counter and prescribed) medications and dosage rates. Make sure to have your primary doctors’ office send or bring with you any medical records, CT, MRI, and/or X-ray that pertains to your visit. In addition bring your insurance card, copay, and arrive 15 minutes early to complete our new patient paperwork.
